Grant Mission: The Local Impact Community Wellness Grant is specifically designed to fund high-impact, preventative programs that address the Social Determinants of Health (SDoH) and reduce chronic disease burdens in vulnerable populations within the MHS service region.


Funding Priorities (2026 Cycle): Proposals must align with one of our three core priorities identified in our latest Community Health Needs Assessment (CHNA):

  1. Behavioral Health Access: Projects expanding mental health and substance abuse services in school or community settings.
  2. Chronic Disease Prevention: Programs focusing on diabetes, hypertension, or obesity management through nutrition and physical activity education.
  3. Maternal and Child Health: Initiatives reducing infant mortality rates or improving pre-natal care access in high-risk zip codes.


Eligible Applicants: This grant is open to 501(c)(3) non-profit organizations, public schools, and local government agencies that can demonstrate an established presence and trust within the target community.


Funding Range: We anticipate awarding approximately 15-20 grants, with individual awards ranging from $25,000 to $100,000 USD for a one-year project period.

Required Outcomes: Projects must include clear, quantifiable metrics for measuring success, such as: change in patient/participant knowledge scores, increase in client engagement rates, or reduction in emergency department utilization for target conditions.


Collaboration Requirement: Preference will be given to proposals demonstrating a strong, multi-sector collaborative approach (e.g., a partnership between a food bank, a local clinic, and a faith-based organization).


Important Note: Funds are restricted to program delivery, evaluation, and directly related staffing. They cannot be used for capital campaigns, lobbying, or general operating expenses.
